Demographic

Please have your current/most updated contact information available during the check-in process.

Insurance

You will be required to present your insurance card and government issued photo ID card at every visit.

Co-payments, Deductibles, Coinsurance

You are required to pay all co-payments (set rate you pay for doctor visits) at the time you check-in. If you have not met your annual deductible, you may have a payment. If you met your deductible, you may still have to pay for medical services which is called coinsurance (percentage of costs you pay after deductible).

No Insurance

If you do not have medical insurance, we will provide you with the office fee before your appointment and you are responsible for full payment at the time of service.

Referrals

HMO medical insurance requires a referral from your primary care physician (PCP) to see an ophthalmologist. It is your responsibility to obtain and bring that documentation before your appointment or during check in. If you do not have the referral, we cannot see you and you will have to reschedule.

Outstanding Balance

Any outstanding balances will be assessed at the time of check-in and should be paid prior to your examination. If you are unable to pay prior to medical examination, we will arrange a payment plan before your examination with the doctor. Balances more than 75 days late after insurance payment may be sent to a collection agency, and you will be responsible for any additional fees.

Returned Checks

Returned checks are subject to a handling fee of $30.00. In the event your account must be turned over for collection, you will be billed and are responsible for all fees involved in that process.

Refraction

A refraction (glasses prescription) is a part of your comprehensive eye exam and is not covered by medical insurance. We do not accept vision insurance currently. We charge $50.00 for refractions, and it is due at the time of service.

No-show Appointments

Please give us 24 hour advanced notice if you are unable to make your appointment. There is no cancellation fee.

Late Appointments

If you are more than 15 minutes late to your appointment, we reserve the right to change your appointment to a walk-in appointment, which means the doctor will fit you into the schedule per their discretion. There may be a longer wait than usual, and you may have to make another appointment if you were scheduled for ancillary testing. You may change your appointment to another date at no charge.

Medical Records & Forms

You must fill out a record request. A copy of your records will incur a fee of 50 cents per page. Give us 2-3 weeks advanced notice before records are expected to your desired location. Please give us 2-3 weeks to fill out medial forms.

Surgery Procedures

Surgery estimates will be due 1-2 weeks prior to your surgery procedure.

Children & Minors

Parents must be present for all minors during the examination and visit. We see minors ages 12 and up. If patients are under the age of 12 years old, we will refer you to the appropriate optometrist or pediatric ophthalmologist for eyecare.

We realize that temporary financial problems may affect timely payment of your account. If such problems do arise, we encourage you to contact us promptly for assistance in the management of your account.

Our staff will make arrangements for you to make monthly payments over an approved term. If you have any questions about the above information or any uncertainty regarding your insurance coverage, please do not hesitate to ask us. We are here to help you.
